MetroPlus Advantage Plan
MetroPlus Advantage Plan (HMO D-SNP) is a Dual-Eligible Special Needs Plan designed for those Medicare beneficiaries who live in New York City, are eligible for Medicare Parts A and B, and receive assistance from New York State Medicaid. Depending on your level of Medicaid eligibility, you may pay as low as $0 for covered services.
MetroPlus Medicare Platinum Plan (HMO) is a plan designed for those Medicare beneficiaries who live in New York City and are eligible for Medicare Parts A and B. When you join our Platinum plan, you will receive your traditional Medicare benefits, plus the extra benefits offered by MetroPlusHealth. Medicare beneficiaries who may not qualify for Extra Help for Part D may like this plan.
MetroPlus UltraCare (HMO D-SNP) is a Medicaid Advantage Plus plan designed for people who are dual eligible for Medicare and full Medicaid, live in New York City, and need long-term care services such as home care and personal care. The plan covers most Medicare and Medicaid benefits, including long-term care or home and community-based services, plus Prescription Drug coverage. Premiums may be as low as $0 per month for those that receive Extra Help paying for their prescription drug premium.

APPOINTMENT OF REPRESENTATIVE
Did you know?
You can ask someone to act on your behalf. If you want to, you can name another person to act for you as your “representative” to ask for an organization determination (coverage decision). There may be someone who is already legally authorized to act as your representative under State law. If you want a friend, relative, your doctor or other provider, or other person to be your representative, print out and complete this Appointment of Representative form in English or Spanish. The form must be signed by you and by the person who you would like to act on your behalf. You must send in or fax a copy of the signed form to 1-212-908-8701.
If you are turning 65 within the next 3 months or you are 65 years of age or older, you may be entitled to additional medical benefits through the Medicare program. If you have Medicaid, you MUST apply for Medicare as a condition of continuous eligibility.
You can apply for Medicare by calling the Social Security Administration (SSA) at 1-800-772-1213 or by applying on-line at: ssa.gov/medicare/. You may be required to submit proof that you have applied for Medicare so please keep your award or denial letter from SSA or your online receipt from SSA.
