Follow these four easy steps to get started.

Create a MetroPlusHealth Member Portal account (or sign in if you already have one).
MetroPlusHealth uses Salesforce for third-party verification.

Once in the Member Portal, click on “Rewards.”

After registering, you will receive your Member Reward Card within 10 – 14 days by mail.

Once you receive your card, start redeeming your rewards

Who is eligible for the rewards program?

MetroPlusHealth members in the following plans are eligible to participate in the Rewards program: Medicaid, Partnership in Care, Enhanced (HARP), Essential plan, Medicare Advantage and Platinum, Ultracare, and MetroPlus Gold.

How do I register?

To access the Rewards program, members must first create and log into their MetroPlusHealth Member Portal account.

Step 1: Create a MetroPlusHealth Member Portal account (or sign in if you already have one).
Step 2: Once in the Member Portal, click on “Rewards.”
Step 3: You will be taken directly to your Rewards portal through Single Sign-On (SSO).

If it is your first time visiting the Rewards portal, you will first be asked to confirm your contact information and confirm participation in the Rewards program, if your plan requires reward registration.

If you have accessed the Rewards portal before, you will go directly to your personalized Rewards Home Page, where you can view your eligible and earned Rewards and OTC information, if applicable to your plan.

Important plan-specific notes:

• Medicare Advantage and UltraCare members will receive their Rewards card immediately after enrollment into the plan, regardless of whether they have accessed the Rewards member portal to confirm their contact information and register their account. Their card will be loaded with OTC funds and any earned rewards

• Medicare Platinum members do not have to register. They will be auto registered upon enrollment to the plan, however their Rewards card will not mail until after an eligible activity is completed.

• Essential Plan members must register their Rewards and OTC account via the member portal or customer service before their card is mailed. Once it is mailed, it will be pre-loaded with available OTC funds and any earned rewards.

• All other plans must register their Rewards account via the member portal or with customer service and complete a rewardable activity before a loaded card is mailed to the member.

How do I earn rewards?

Members can view their eligible rewards by navigating to “My Rewards” within the Member Rewards portal. This section displays the rewardable activities available for each member based on their specific plan.

For most plans, once a member completes a healthy activity that qualifies for a reward, and the activity is processed by MetroPlusHealth, the reward will be automatically deposited into the member’s Rewards account. No additional action is required.

For members enrolled in the Gold or Essential Plan, there will be a “Complete Activity” button next to certain rewardable activities. In these cases, members will be prompted to self-attest to complete the activity before the reward can be processed.

Where can I use my Rewards and Flex card?

The Rewards Card is a point-of-sale card that can be used at Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar, Amazon and numerous independent groceries and drug retailers.

For the full list of retailers, please refer to your Member Rewards Portal, which is accessed by logging into the MetroPlusHealth Member Portal and navigating to the Rewards page.

Once in the Rewards portal:

Click the “Shop” tab.
Select “Retail Lookup.”
Enter your ZIP code to see all participating retailers where the card can be used.

Members can also shop online. Under the “Shop” tab, the portal displays available online shopping options where Reward and OTC funds can be used.

Within the Member Portal, members can also view and track their reward and OTC balances, as well as their past transactions.

When should I expect to receive the card?

The timing for receiving your Rewards or Flex card depends on your Line of Business:

MetroPlus Advantage Plan (HMO D-SNP) and MetroPlus UltraCare (HMO D-SNP)
Members will receive their card shortly after enrollment. The card will be pre-loaded with their OTC benefit dollars and can be used right away. When a member completes a qualifying health activity, any earned Reward dollars will also be automatically loaded onto the same Flex card.

Essential Plan:
Members must first register for the OTC and Rewards program via the Member Portal. After completing registration, a card with their OTC dollars will be mailed. Once the member completes a qualifying health activity, Reward dollars will also be loaded to this card.

All other plans (Medicaid, Enhanced (HARP) Plan, Partnership in Care (PIC) HIV Special Needs Plan (SNP), Gold): Members will receive their card after they both:

Complete a qualifying health activity, and
Register for the program through the Member Rewards Portal.
- Medicare Platinum members do not have to register but do need to complete a qualifying health activity.

The card will then be mailed with the reward amount associated with the completed activity.

When I receive my member rewards card will there be a default value?

When the card is mailed, it is pre-loaded with the value of the earned Reward dollars or OTC funds.

MetroPlus Advantage Plan (HMO D-SNP) and MetroPlus UltraCare (HMO D-SNP):
Your card will arrive with your Flex (OTC) wallet funds already loaded, even if you have not completed a rewards activity yet. Once you complete a qualifying health activity, any earned Reward dollars will be added to the same card automatically.

Essential Plan:
After you register for the OTC and Rewards program, your card will be mailed with your Flex (OTC) wallet funds pre-loaded. Reward dollars will be added after you complete eligible health activities.

All other plans (Medicaid, Enhanced (HARP) Plan, Partnership in Care (PIC) HIV Special Needs Plan (SNP), Gold, and Medicare Platinum*):
Cards for these plans do not include a default balance. The card will be mailed after:

You complete a qualifying rewards activity
You register through the Member Rewards Portal
*Platinum members do not need to register.

The card will arrive with the reward amount earned from your first completed activity.

Do I have to notify anyone after completing a healthy activity?

Members who are part of Medicare, Medicaid, Enhanced (HARP), and Medicare plans do not need to notify anyone after completing a rewarded activity. MetroPlusHealth automatically receives confirmation from the provider and will load rewards accordingly.

Members who are part of Gold and Essential Plan require self-attestation for certain activities, which they can do right in the Member Portal, under the “My Rewards” tab.

To do this: Members should access the MetroPlusHealth Member Rewards Portal, and navigate to the “My Rewards” tab.

If self-attestation is required for your plan or activity, you will see a “Complete Activity” next to the specific activity. You may be asked to provide information about the activity you complete before the reward can be processed.

Who should I call to activate the card and check balance?

Your member’s Rewards card works just like a debit card and therefore needs to be ‘Activated’ before using. Members can call 1-833-963-8764 to activate your card.

To check your balance and see your historical transactions, log into the MetroHealthPlus Member Rewards portal.

Who should I call to report my card stolen and/or lost?

Members can call 1-833-963-8764.

Are there age restrictions on who earns the reward?

Yes. There are some age restrictions on earning rewards. You can see what the age parameters are for each activity in your Member Rewards Portal, by highlighting the icon. There will be a pop-up that shows the eligibility parameters.

Additionally, dependents enrolled on a Commercial plan are not eligible to earn rewards.

Is there one card per household, or does each member of the family get their own card(s)?

Currently, each member will receive a card. Parents or guardians will receive a card on behalf of their enrolled child (18 and under).

How soon after I complete an activity do I see the rewards on my card for me to spend?

Rewards are typically available within 60–90 days, depending on how quickly your provider submits the claim to MetroPlusHealth for processing.

If you report (self-attest) your activity on the MetroPlus Member Rewards Portal, your reward dollars will typically be available within one week.

Is the member required to report or submit anything after completing the activity, or does the system automatically track the activity and load the rewards to the card?

Members have to register on the Member Portal and click My Rewards. You only have to register once, and you do not have to notify anyone. Your rewards activity, i.e.: PCP Annual visit will be automatically tracked by MetroPlusHealth.

For most MetroPlus Health plans, you do not need to do anything after completing a healthy activity and registering for the Rewards program.
The system automatically tracks eligible activities, and your reward will process and load to your Rewards card once the activity is verified.

For Gold members and EP members, you must self-attest to some of your activities. Once self-attestation is submitted and processed, the reward will be loaded to the card.

When do these rewards expire?

For active members, earned rewards expire 365 days after the earning year. For members who are no longer enrolled with the plan, their earned rewards will expire immediately.

Note: for 2026 rewards wallet will expire 4/30/2027

For active members, Rewards earned in 2025, will expire 12/31/2026.
If you transfer to a new MetroPlusHealth plan, you will retain your rewards until the expiration date.
If you leave MetroPlusHealth, any remaining Reward or OTC dollars will be forfeited.

How can I spend the funds on my rewards card?

The Rewards Card is a point-of-sale card that can be used at Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar, Amazon, and numerous independent groceries and drug retailers.

You can use your Rewards card at approved retailers to pay for eligible items, both in-store and online. To see the full list of participating retailers and where your card can be used, visit the Metro Member Rewards Portal. The portal is your one place to view your balance, track rewards, and find where to spend your funds.

What if I don’t have access to a smart phone/internet and need to register manually?

If I do not have access to the internet, can a physical catalog be mailed to me?

Unfortunately, there is not a physical catalog because of the large amount of eligible items at so many different merchants.
