You will receive your rewards on a MetroPlusHealth branded Rewards Card with your name on it.

You can use your Rewards Card at Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar, AHOLD Brand Stores, Dollar General and participating Independent Drug and Grocery Stores.

How do I register?

Registration is simple—enroll in the MetroPlusHealth member portal and click on the “My Rewards” tab. Complete a quick registration and you will automatically be connected to the Rewards portal.

How do I earn rewards?

Each plan (Medicaid, Essential Plan, Medicare, etc.) has several eligible rewards. Read through the rewards that you are eligible for under your plan. Once you complete these healthy rewards activities (for example, completing an annual PCP visit or completing a preventive screening such as a breast cancer screening) your rewards earning will be automatically added to the Rewards portal.

How do I redeem rewards?

As soon as you register on the portal and have completed your first activity, you are eligible to receive a Visa debit card. This debit card will need to be activated and will be yours for as long as you remain a MetroPlusHealth member. This card will automatically update every time you complete an eligible activity and dollars will be added that are yours to spend.
