You are already delivering excellent care to your MetroPlusHealth members as part of your existing contractual arrangement. The Pay-For-Performance (P4P) Program provides an additional incentive opportunity to reward you even further for the high-quality care you are provide.

Why Your Panel Size Matters
Providers must have at least 150 attributed members (group TIN or solo NPI) to participate and earn P4P incentives.
P4P Earning Tiers:
150–199 members → Eligible for 50% of the available reward
200+ members → Eligible for 100% of the available reward
Growing your panel improves eligibility and increases total P4P earnings.
To help maintain your panel size, remember to support members with recertification so they remain active and continuously attributed to your practice. MetroPlus can provide each member’s recertification date so you can proactively assist with outreach.

Ensuring Your Panel Is Accurate
Ask patients to confirm you as their PCP on their MetroPlus ID card. If they cannot, please have them call the Member Services number found on the back of the card.
Keep your demographic details current on the Provider Portal or email [email protected].

Engage Your Attributed Members Early and Often
Download your panel roster via the Provider Portal (help available at [email protected]).
Aim to see new members within the first two months.
Reach out to inactive members to bring them in for preventive or routine care.
