Your Personalized Care Plan
When you join MetroPlusHealth Managed Long-Term Care, you’ll get a dedicated Care Manager who will:

• Work closely with you, your family, and your doctor
• Create a care plan tailored to your unique needs
• Address your medical, physical, emotional, and social requirements
• Review your plan every six months (or sooner, if needed)
• Provide you with a copy of your care plan
