You’re eligible if you are:
- Are 21 or older
- Have Medicaid
- Live in any NYC borough (Bronx, Brooklyn, Manhattan, Queens, or Staten Island)
- Expected to require at least one of the community based long term services and support (CBLTSS) for a continuous period of more than 120 days from the date of enrollment: Can safely stay in your home with support
