$50 for colonoscopy in current year OR $10 for colonoscopy within 9 years prior. OR $15 for FIT-DNA in current year OR $5 for FIT-DNA within 2 years prior. OR $10 for FOBT, FIT

Diabetes Eye Exam$30 per calendar yearAge: 18+Completion of diabetic retinal eye exam

To be eligible for this reward, the requirement is clinical eligibility – diagnosis of type 1 or type 2 diabetes.

Flu Shot$10 per calendar yearAge: 18+Complete annual flu vaccine
Health Assessment (HRA-MA)$10 per calendar yearAge: AnyComplete a MPH Medicare Health Assessment (Medicare) for CMS compliance
New Member PCP Visit$20 per calendar yearAge: AnyComplete visit with PCP within 90 days of enrollment
Medicare Personal Health Assessment$50 per calendar yearAge: 21 – 85Complete a Personal Health Assessment and earn $50 in rewards (eligible members only). This reward is available once per calendar year.

RECEIVE YOUR REWARDS ON A REWARDS CARD

You will receive your rewards on a MetroPlusHealth branded Rewards Card with your name on it. You can use your Rewards Card at Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar, Stop & Shop, Dollar General and participating Independent Drug and Grocery Stores.

You can redeem your rewards for items like over-the-counter medicines, sport drinks, personal care items, lip balm and more at participating retailers.
