If you’re planning to enroll in a MetroPlusHealth Medicare plan, you’ll need a few things. First, you have to be enrolled in Medicare Parts A and B. Then, you’ll have to prove your identity and that you meet the eligibility requirements, which may require documentation such as your birth certificate, military discharge papers or your W-2 from the past year.

Below, we’ll go over how to apply for Medicare, and all the information and documents you need to do so.

Information needed for Medicare enrollment

To be eligible for Medicare, you must be age 65 or older, or have a disability, end-stage renal disease or ALS (Amyotrophic Lateral Sclerosis; also known as Lou Gehrig’s Disease). In some circumstances, such as if you’re receiving Social Security retirement or disability benefits, Medicare and Social Security may already have your information, but if not, you’ll need to prove the following:

  • Citizenship
  • Age
  • Work/income history
  • Military service if applicable
  • Social Security number
  • Name and Social Security number of current and/or previous spouses
  • Date and place of any marriages/divorces
  • Names and ages of any minor children you have, or children up to 19 who are still in high school
  • Names/ages of any children you have who was diagnosed with a disability before age 22
  • Whether you’ve applied for Social Security benefits in the past, or if someone has on your behalf
  • Name/address of any employer you’ve had in the past two years
  • The amount of money you’ve earned in the past two years
  • The date of any military service you served before 1968
  • Information about work you or a spouse has done for the railroad industry, information about any Social Security work credits you’ve earned in another country, and/or information on any federal pension you receive now or in the future

If you don’t have the original copies of your supporting documents, you’ll have to apply for new originals.

Supporting documents for Medicare enrollment

To prove you meet the eligibility requirements, you will likely need supporting documentation, such as:

  • Original birth certificate to prove date and place of birth
  • Proof of citizenship: If you were born outside the U.S., you can prove your citizenship using your U.S. passport, a naturalization certificate, a certificate of citizenship, or a U.S. consular report of birth
  • For non-U.S. citizens, you’ll need to prove legal residency with your permanent resident card (green card), and your admission-departure record
  • Discharge papers from any military service before 1968, if applicable

Enroll in Medicare

Once you have all your information and documentation prepared, all that’s left to do is enroll in Medicare.First, you’ll need to enroll in Medicare Parts A and B at Medicare.gov. Then, you can enroll in one of MetroPlusHealth’s three Medicare plans: MetroPlus Advantage Plan, MetroPlus Platinum Plan and MetroPlus UltraCare Plan.

The Platinum Plan is one of Medicare’s regular Part C plans, while the Advantage Plan is a dual eligible plan for those with Medicaid and Medicare. Ultracare is also a dual eligible plan for people with certain disabilities that require home care.
