Signing up for health insurance can seem daunting – but it’s easier if you know what to expect. Let us walk you through our enrollment process for the MetroPlusHealth Essential Plan to help you feel confident and ready to sign up.

How to enroll in MetroPlusHealth’s Essential Plan

As you may expect, you’ll need to provide contact information during enrollment. This includes your address, phone number and email address, so be sure you’re prepared with that information.

Next, you’ll want to gather the documents you need to provide proof of identity and date of birth, proof of residency, and proof of current income. Documents you may be able to use include:

Proof of identity and date of birth:

  • Identification such as social security number or other documentation from the United States Citizenship and Immigration Services for documented immigrants. Note that immigration status does not affect eligibility with MetroPlusHealth.
  • Drivers license
  • Passport
  • Birth certificate
  • Marriage records
  • Baptismal or other religious certification

Proof of residency:

  • Utility bill
  • Bank statement
  • Lease/rent receipt
  • ID card with address
  • Mail with name and date

Proof of income:

  • Paystubs
  • Tax returns
  • Benefit check
  • Bank or broker statements
  • Military check stub

Once you have your information ready, you’ll need to call MetroPlusHealth. The member services team is available weekdays 8 a.m. – 6 p.m., and Saturdays 9 a.m. – 5 p.m. at 1.855.809.4073 (TTY:711).

How long does enrollment take?

While enrolling in the Essential Plan isn’t a super lengthy process, you’ll still want to set aside some time for it so that you aren’t rushed and can ask questions you have about care plans, healthcare payment, income requirements or anything else. Try and commit around 30 minutes for enrollment so that you and the member services team can make sure all your information is accurate and you have all the details you need. If you have many family members to enroll, you may want to give yourself more time.
