ESSENTIAL PLAN FULL LIST OF ACTIVITIES AND REWARDS

HEALTHY ACTIVITY	REWARD AMOUNT	QUALIFYING AGE	REWARD RULES
Asthma Medication Management	$25 per 90-day fill, $100 max	Age: 21-64	Refill asthma controller medication. To be eligible for this reward, the requirement is clinical eligibility – diagnosis of persistent asthma, for prescribed long-term controller medication only and does not include rescue medications.
Breast Cancer Screening	$25 per calendar year.	Age: 50-74	Complete mammogram
Cervical Cancer Screening	$25 per calendar year	Age: 24-64	Complete cervical cancer screening
Chlamydia Screening	$25 per calendar year	Age: 21-24	Complete chlamydia screening
Colon Cancer Screening	$25 per calendar year	Age: 45-75	Complete colon cancer screening
Diabetes Eye Exam	$25 per calendar year	Age: 21-75	Completion of diabetic retinal eye exam. To be eligible for this reward, the requirement is clinical eligibility – diagnosis of type 1 or type 2 diabetes.
Flu Shot	$25 per calendar year	Age: 21+	Complete annual flu vaccine
Kidney Health Monitoring	$25 per calendar year	Age: 21 – 85	Complete eGFR and uACR testing.
New Member PCP Visit	$25 per calendar year	Age: 21+	Complete visit with PCP within 90 days of enrollment.
Statin Medication Management	$25 per 90-day fill $100 dollar max	Age Males: 21-75 Age Females: 40-75	Refill your statin medication as prescribed.
Semi-Annual PCP Visit	$25 in each half of the year	Age: 21+	Complete primary care physician (PCP) visits. $25 in each half of the year. A minimum of 3 months must occur between semi-annual visits and/or any new member visit.

RECEIVE YOUR REWARDS ON A REWARDS CARD

You will receive your rewards on a MetroPlusHealth branded Rewards Card with your name on it. You can use your Rewards Card at Duane Reade, Walgreens, CVS, Rite Aid, Family Dollar, Stop & Shop, Dollar General and participating Independent Drug and Grocery Stores.

You can redeem your rewards for items like over-the-counter medicines, sport drinks, personal care items, lip balm and more at participating retailers.
