RE: Updated Rates for Health Home Plus 2.84% COLA, Retro-effective April 1, 2024

IMPACTED PLANS: Medicaid, HIV SNP, and HARP

TO: Health Homes Providers

DOH has released guidance to notify that (retro) effective April 1, 2024, the Medicaid rates for Health Home Plus Care Management have been adjusted to include the 2.84% Cost of Living Adjustment (COLA) increase, per the 2024-2025 New York State enacted budget.

MetroPlusHealth has configured their systems to process and pay the adjusted rates for these services for previously paid claims billed for Health Home Plus Care Management with dates of service on or after April 1, 2024.
