If you’re looking for a health insurance plan, you may want to know about income qualifications, any payment requirements, and options for immigrants. Signing up for health insurance can seem scary – but it’s easier if you know what to expect. Though enrollment details may vary between plans, we’ll guide you every step of the way.
What do our plans cover?
Our plans cover essential health care needs, from preventive visits and immunizations to hospitalization, lab tests, and prenatal care. Individual benefits vary based on your plan, but you can typically count on things like preventive care, immunizations, specialist visits, and hospitalizations.
What do I need to enroll for MetroPlusHealth plans?
You will need contact information, including your address, phone number and email address, so be sure you have that.
Next, you’ll need to provide proof of identity and date of birth, proof of residency, and proof of current income. Documents you may be able to use include:
Proof of identity and date of birth:
- Identification such as social security number or other documentation from the United States Citizenship and Immigration Services for documented immigrants. Note that immigration status does not affect eligibility with MetroPlusHealth.
- Drivers license
- Passport
- Birth certificate
- Marriage records
- Baptismal or other religious certification
Proof of residency:
- Utility bill
- Bank statement
- Lease/rent receipt
