MetroPlusHealth offers Medicare health plans to be included as part of your overall health insurance coverage, however you are not required to enroll in MetroPlusHealth to receive your Medicare coverage.
Initial Medicare enrollment lasts for 7 months, starting 3 months before you turn 65, and ending 3 months after the month you turn 65. If born on the 1st of the month, enrollment is 4 months before you turn 65 and ends 2 months after the month you turn 65.
MetroPlusHealth Plan, Inc. is an HMO, HMO D-SNP plan with a Medicare contract. MetroPlusHealth Plan, Inc. has a contract with New York State Medicaid for MetroPlus UltraCare (HMO D-SNP) and a Coordination of Benefits Agreement with the New York State Department of Health for the MetroPlus Advantage Plan (HMO D-SNP). Enrollment in MetroPlusHealth Plan, Inc. depends on contract renewal.
