UPDATE: Adjusted Rates for Health Home Plus Care Management - 4% COLA

IMPACTED PLANS: Medicaid, HIV SNPs, and HARP
DOH has released guidance to notify that (retro) effective April 1, 2023, the Medicaid rates for Health Home Plus Care Management have been adjusted to include the 4% Cost of Living Adjustment (COLA) increase, per the 2023-2024 New York State enacted budget.

MetroPlusHealth systems configuration is in progress to process and pay the updated rates for these services and issue retro-active payment adjustments to providers for any Health Home Plus Care Management claims previously paid with dates of service on or after April 1, 2023.
