Did you know?
You can ask someone to act on your behalf. If you want to, you can name another person to act for you as your “representative” to ask for an organization determination (coverage decision). There may be someone who is already legally authorized to act as your representative under State law. If you want a friend, relative, your doctor or other provider, or other person to be your representative, print out and complete this Appointment of Representative form in English or Spanish. The form must be signed by you and by the person who you would like to act on your behalf. You must send in or fax a copy of the signed form to 1-212-908-8701.

If you are turning 65 within the next 3 months or you are 65 years of age or older, you may be entitled to additional medical benefits through the Medicare program. If you have Medicaid, you MUST apply for Medicare as a condition of continuous eligibility.
You can apply for Medicare by calling the Social Security Administration (SSA) at 1-800-772-1213 or by applying on-line at: ssa.gov/medicare/. You may be required to submit proof that you have applied for Medicare so please keep your award or denial letter from SSA or your online receipt from SSA.

Members can create a new account or sign in to an existing account on their member portal and follow the instructions to request a hard copy directory.
If you want a Provider/Pharmacy Directory or Evidence of Coverage (EOC) mailed to you or if you need help finding a network provider/pharmacy, please call 833-965-1526 (TTY: 711). You may also request the directory or EOC by filling out our Contact form. You can always access our online directory on our Provider Directory page or your EOC on the Get the Details page.

Are you receiving care at an NYC Health + Hospitals facility?
Manage Your Health Online With MyChart. Members who receive care at NYC Health + Hospital facilities now have access to MyChart. MyChart is a free secure portal you can use to manage your health information.
MyChart allows you to:
- Message your primary care provider’s office.
- Request refills of your prescription drugs.
- Access your test results.
- Schedule appointments.
MetroPlus Health Plan is a HMO, HMO SNP plan with a Medicare contract. MetroPlus Health Plan has a contract with New York State Medicaid for MetroPlus UltraCare (HMO D-SNP) and a Coordination of Benefits Agreement with the New York State Department of Health for the MetroPlus Advantage Plan (HMO D-SNP). Enrollment in MetroPlus Health Plan depends on contract renewal.
